Unified Group Services, Inc. (Unified) of Anderson, Indiana administers the Medical benefit plan offered through Muncie Community Schools’ Group Health Plan.  Unified will process and answer any claims questions that you or your providers may have. 

Encircle with Encore Health Network will be your Preferred Provider Organization (PPO).  Remember, you must utilize Encircle with Encore Health Network providers for all

healthcare needs or you will receive a reduction in benefits.  The Encircle with Encore Health Network provider inquiry line is 888-446-5844 and their website is http://www.encoreconnect.com.

You are required to contact Med-Cert, Inc. to pre-notify all Inpatient Admissions and select outpatient procedures which are listed below; at 800-356-7126.  It is ultimately the patient’s responsibility to assure that pre-notification has occurred.

Your pharmacy network for prescription drugs will be EnvisionRxOptions.  You will find most pharmacies in your area are members of the EnvisionRxOptions Network, and you may also access the website (http://www.envisionrx.com) to check pharmacy providers locally and in all 50 states.
