Regardless of the nature or severity, all injuries incurred while on the job must be reported to your supervisor immediately. Injured employees needing medical treatment will obtain the appropriate “Treatment Authorization” form signed by a building administrator and visit one of the three treatment facilities. Please note: IU Health Ball Memorial Hospital should only be visited for life threatening emergencies or if the other treatment facilities are closed.

All employee injuries are to be reported to Indiana Insurance Company within one (1) business day (24 hours) of the injury, not counting weekends and holidays. MCS nurses are responsible for reporting all injuries and submitting the First Report of Employee Injury. The nurse does not have to witness or treat an injury to submit the First Report of Employee Injury. It is the injured employee’s responsibility, or anyone else with knowledge of the injury, to provide basic information to the appropriate school nurse.
If the initial physician’s visit results in an unrestricted release to return to work, the injured employee is to report back to duty immediately. If and when the physician releases the injured employee to return to alternate light duty or restricted duty, the injured employee must provide the appropriate medical documentation stating the restrictions and report as follows:
- Custodial/Maintenance to Service Center (Mr. Justice)
- Food Services to Food Services Office (Mrs. Baierwalter)
- All others to Immediate Supervisor/Building Administrator
